Health Declaration Form

A Herd Health Declaration (HHD) form must be filled out for all animals entered into Society Sales.  These must be returned to the Society along with your sale entry forms.

There are a number of formats in which we can accept HHD submissions.

Firstly, a blank copy of a HHD form can be downloaded below. This form is in a form-fillable .pdf format, and can be completed on a computer. If you prefer, you may instead print this form off and fill in your details manually.

Please return completed HHD forms to the Society office either via email to charlotte@charolais.co.uk, or via post to The British Charolais Cattle Society, Avenue M, Stoneleigh Park, Kenilworth, Warwickshire, CV8 2RG.

Alternatively, you can fill in our web form below. After clicking the “Submit” button, your form will be automatically emailed to the Society. Please note that fields marked with an asterisk (*) are compulsory.

Any animals being sold which are not homebred will retain its Johnes level from the herd it was bred in. If the vendor cannot prove this via the breeder, then individual animals will be declared as Johnes level 5.

Vaccination dates can be future dates at the time of submitting your declaration as long as the final dose is given a minimum of 3-4 weeks prior to the sale date.

By submitting this form you are certifying that the information is correct as at the date of submission and permit the BCCS to verify the details with the relevant CHeCS Health Scheme. The responsibility for the accuracy of the information rests solely with the breeder and not with the British Charolais Cattle Society. If the herd is not CHeCS accredited for BVD a BVD Antigen Blood Test MUST be taken and a copy of the certificate MUST be taken to the sale and given to the auctioneers.
